Please complete the Registration Form and submit it, by e-mail or fax to the Conference  Secretaries,  by 1^st October 1998  together with your Events Booking Form and Payment Sheet (earlier submission gives priority). 
Please fill out also the Hotel Reservation Form, and fax it to our travel agent and send to us the electronic form version too (check 'How to Book your Hotel Accomodation' section).
